Полиморфизм гена ADRB2 как фактор наследственной предрасположенности к развитию бронхиальной астмы и ответа на терапию сальбутамолом

Цель — исследовать ассоциацию полиморфизма C79G гена АDRB2 с риском развития бронхиальной астмы (БА) у детей, проживающих в различных условиях загрязнения окружающей среды, а также смоделировать 3D структуры изоформ белка β2-адренорецептора, которые кодируются полиморфными вариантами гена АDRB2 (A46G, C79G, і C491T) in silico для прогноза структурно-функциональных свойств, которые могут влиять на взаимодействие с сальбутамолом.
Пациенты и методы. Обследовано 114 детей в возрасте от 3 до 18 лет с БА средней тяжести, контролируемого течения, которые методом рандомизации были разделены на две группы: группу I (дети из условно чистого региона Киева и Киевской области) и группу II (дети из экологически загрязненного региона). Группу контроля составили 86 неродственных здоровых взрослых из разных регионов Украины. Полиморфный вариант гена АDRB2 (C79G) исследован методом аллельспецифической полимеразной цепной реакции. Компьютерное моделирование 3D структуры изоформ белка β2-адренорецептора проведено с использованием веб-серверов SWISS-MODEL и I-TASSER, молекулярный докинг проведен с использованием программы Auto Dock Vina.
Результаты. Установлено, что частота носителей полиморфного варианта 79G гена ADRB2 статистически достоверно выше (p<0,05) в группе обследования II (69,4%) по сравнению (55,8%) с частотой в контрольной группе. По результатам анализа смоделированной пространственной структуры белка ADRB2, замены p.16Arg>Gly и p.27Gln>Glu находятся в N-концевой последовательности и могут влиять на взаимодействие с белками-партнерами, в свою очередь, аминокислотная замена p.164Thr>Ile локализирована вблизи сайта связывания с лигандами и может снижать аффинность сальбутамола к соответствующему мутантному рецептору.
Выводы. Полиморфный вариант 79G гена ADRB2 может рассматриваться в качестве фактора наследственной предрасположенности развития БА в условиях антропогенной нагрузки окружающей среды. Мононуклеотидная замена 491C>T гена ADRB2 может рассматриваться в качестве фармакогенетического маркера плохого ответа пациента на лечение сальбутамолом.
