Миниинвазивная коррекция стеноза дыхательных путей у детей

Аннотация

Цель — оценить эффективность миниинвазивных методов коррекции стенозов дыхательных путей у детей.

Материалы и методы. Проведено ретроспективное исследование пациентов, которым диагностирован врожденный или приобретенный подсвязочный ларинготрахеальный стеноз, врожденный стеноз трахеи, обусловленный полными хрящевыми кольцами, врожденный стеноз промежуточного бронха и был проведен по меньшей мере один сеанс эндоскопического лечения.

Результаты. В исследование включен 41 пациент в возрасте от 3 недель до 15 лет (22,6±6,2 месяца), 24 (58,5%) пациента были моложе 1 года. У 11 пациентов выявлен врожденный, у 23 — постинтубационный подсвязочный стеноз, у 4 — врожденный стеноз трахеи, обусловленный полными хрящевыми кольцами, у 3 — врожденный стеноз промежуточного бронха. У 39 (95,1%) пациентов миниинвазивные эндоскопические процедуры проведены как первичное лечение, в 6 случаях ими дополнена хирургическая коррекция. В случае ларинготрахеального стеноза I, II и III степени эффективность первичного применения миниинвазивных методов составляла 100%, 86,4% и 40% соответственно, при полных хрящевых кольцах и стенозе промежуточного бронха — 100%.

Выводы. Общая частота положительных результатов при первичном применении миниинвазивных методов коррекции стенозов дыхательных путей составила 84,6%, что соответствует данным современной литературы.
